Do I need an appointment?
We are happy to accept walk-in patients at all our South Louisiana Primary Care (SLPC) locations. We do highly recommend having an appointment to reduce wait times - simply call (225) 774-0733 to check-in.
What are the hours of operation?
All South Louisiana Primary Care (SLPC) locations are open Monday–Friday from 7:00 am to 4:30 pm.
What do I need to bring with me for my appointment?
If you have insurance, please bring your insurance card at all appointments. Also bring your medication list with name, dosage, and directions for use.
For patients 17 years of age or older, you will need a valid picture ID. For patients under the age of 17 years of age, a guardian must be present at the time of treatment and provide a valid picture ID.
What types of insurance does South Louisiana Primary Care (SLPC) accept?
South Louisiana Primary Care (SLPC) accepts all Louisiana Medicaid and most Medicare policies, most major Private and Commercial Insurance policies and Self Pay. A complete list of the types of insurance we accept can be found on our Insurance Page under Patient Resources.
Can I complete my paperwork prior to my visit?
Yes! You can print and complete the New Patient forms prior to your visit and bring them with you for your scheduled appointment. Paperwork not completed ahead of time may result in longer wait times.
Can South Louisiana Primary Care (SLPC) treat children?
Yes! Our SLPC providers are trained to treat patients from five (5) years of age and up.
Does South Louisiana Primary Care (SLPC) offer telemedicine visits?
Yes! Once our patients personally meet with one of our providers, SLPC offers telemedicine visits for them.
What if I need to see a specialist?
If one of our SLPC providers determines you need to be seen by a specialist, we will make a recommendation for you.
Who do I contact regarding questions about my bill?
If you have questions or concerns about your medical bill for any of our locations, please call (225) 774-0733. Our billing department may assist you Monday–Friday from 7:00am to 4:30pm.
